The Affordable Care Act (Obamacare) was designed so that all plans purchased in the Marketplace (ACA exchanges) cover a base set of essential health benefits (EHB) which consist of the following:
- Ambulatory patient services (outpatient care you get without being admitted to a hospital)
- Emergency services
- Hospitalization (like surgery and overnight stays)
- Pregnancy, maternity, and newborn care (both before and after birth)
- Mental health and substance use disorder services, including behavioral health treatment (this includes counseling and psychotherapy)
- Prescription drugs
- Rehabilitative and habilitative services and devices (services and devices to help people with injuries, disabilities, or chronic conditions gain or recover mental and physical skills)
- Laboratory services
- Preventive and wellness services and chronic disease management
- Pediatric services, including oral and vision care (but adult dental and vision coverage aren’t essential health benefits)
- Birth control coverage
- Breastfeeding coverage

Trump issued an executive order entitled "Presidential Executive Order Promoting Healthcare Choice and Competition Across the United States". Here are point-by-point excerpts of the order:
Section 1. Policy. (a) It shall be the policy of the executive branch, to the extent consistent with law, to facilitate the purchase of insurance across State lines and the development and operation of a healthcare system that provides high-quality care at affordable prices for the American people.

Federal Judge Rosemary Collyer ruled in favor of the U.S. House of Representatives in it's lawsuit against the Obama administration regarding the "cost sharing reduction payments" part of Obamacare.
"Cost sharing reduction payments" are payments made by Obamacare to insurance companies so the companies can charge lower rates to lower-income customers so the insurance is affordable.
Today's court ruling says that since the money for those payments has not been appropriated by Congress, the Obama administration does not have the power to spend that money. The Obama administration argues that no appropriation is needed because those payments are already guaranteed by Obamacare in the same section of the law (Section 1402)) that defines tax credits.
Today's ruling does not go into effect immediately, allowing the Obama administration time to appeal it.

On NBC's Meet the Press, host Chuck Todd interviewed Speaker of the House Paul Ryan. In the interview, speaking about Obamacare Ryan said the following:
It's a law that is not working. It's a law that's depriving people's choices. It's a law that's making families pay double digit premium increases. It's a law that's leading Medicare into a rationing scheme. That's not going to work for seniors.
According to a PolitiFact article entitled "Paul Ryan's shaky claim that Obamacare is 'making families pay double-digit premium increases'", Ryan's claim of "double digit premium increases" is only "half true". While it is true that some families have seen double-digit increases, according to the U.S. Department of Health and Human Services the national average increase is 7.5 percent for the 37 states using healthcare.gov. When you factor in premium tax credits and enrollment figures the national average is considerably less than 7.5 percent.
According to PolitiFact, Ryan's statement that "It's a law that's making families pay double digit premium increases." is misleading because according to experts rate increases are more likely the result of insurers underestimating how sick enrollees would be, rather than something inherently wrong with Obamacare itself.

By a vote of 52-47, the U.S. Senate passed legislation to repeal the ACA and defund Planned Parenthood. The legislation now goes to the U.S. House of Representatives. The legislation was able to pass with a simple majority (instead of the usual 60 votes needed to avoid a filibuster) because it is classified as a budget reconciliation measure, which is not subject to a filibuster.

According to data released October 1, 2015, ACA insurers lost money on ACA insurance plans for the year 2014.
The loss is directly related to a shortfall in what's know as the "risk corridors" program/feature of the ACA. Risk corridors is a temporary 3-year program put in place to help accommodate the anticipated high rate of insurance claims as people initially signup for the ACA. Under risk corridors, insurers with less claims help subsidize insurers with more claims. In other words, profitable insurers help cover the costs of unprofitable insurers. The data for 2014 shows that unprofitable insurers requested considerably more reimbursement money than the subsidies provided by profitable insurers, in the amount of about $2.5 billion.
Because of a policy rider included in the 2014 year-end 1,603-page "Cromnibus" U.S. spending bill, the U.S. Department of Health and Human Services (HHS) can no longer get money from other places to help makeup the shortfall in the risk corridors program.

In Washington, D.C., federal Judge Rosemary Collyer ruled that the lawsuit brought against Obamacare by the U.S. House of Representatives is legitimate and the case can move forward.
The lawsuit challenges Obamacare's use of subsidies to help low-income people afford insurance coverage. Essentially, the lawsuit claims the funds used for federal subsidies paid to health insurance companies were not officially approved by Congress.
This lawsuit is the third major legal challenge of the Affordable Care Act.

On March 4 (next week), the U.S. Supreme Court will hear the case of King v. Burwell which challenges the wording and intent of one reference in the ACA. That reference, used many times in the text of the ACA law, is "through an Exchange established by the state". The specific references being challenged are those that relate to tax-credit subsidies which help make insurance acquired through exchanges affordable for the consumer.
At present, 17 of the 50 U.S. states have their own state (state-run) exchanges. The other 33 states have either a federal exchange or plan to have a partnership exchange. The Supreme Court decision in King v. Burwell will affect only those states without state exchanges.
Plaintiffs say that because the wording of the law reads "established by the state", those references do not apply to the other 33 states, and therefore consumers signing up through exchanges in those states are not eligible for subsidies.
Defendants (the Obama administration) say the references are intended to apply to all states, and that other parts of the law make it clear that subsidies are intended for consumers in all states.
If the Supreme Court decides for the plaintiffs, millions of consumers might lose their subsidies, making it more expensive for them to purchase healthcare through exchanges.

By a vote of 5-4, in the case of Burwell v. Hobby Lobby, the U.S. Supreme Court ruled that certain for-profit closely-held businesses (where 50% of the company stock is held by five or fewer people) in which the owners have clear religious beliefs, the businesses cannot be required to pay for certain types of contraceptives for their employees.

The U.S. Supreme Court, in the case of National Federation of Independent Business v. Sebelius, made landmark decisions about the Affordable Care Act
Essentially, the case questioned three different parts of the Affordable Care Act.
The first question was about the individual mandate that requires most Americans must acquire insurance or pay a penalty, and whether that mandate falls within the power of the U.S. Congress to levy taxes. On this point the Supreme Court decided "yes" by a vote of 5-4 (liberal Justices Ruth Bader Ginsburg, Stephen G. Breyer, Sonia Sotomayor, and Elena Kagan, along with conservative Chief Justice John G. Roberts Jr. in favor - conservative Justices Antonin Scalia, Anthony Kennedy, Clarence Thomas, and Samuel Alito against).
The second question was whether the individual mandate is justified by the power of the U.S. Congress to regulate interstate commerce. On this point the Supreme Court decided "no" by a vote of 5-4 (all four liberal justices in favor, all four conservative justices and Chief Justice John G. Roberts Jr. against).
The third question was about Medicaid expansion and whether it was within the power of the U.S. Congress to coerce states into accepting the expansion or lose Medicaid funding all together. On this point the Supreme Court decided "no" by a vote of 7-2.
